It will help you understand your feelings, thoughts and behaviours, and what to do about them.

This Hope Programme is a 6 to 8 week course for people living with multiple long-term conditions, starting on a set date. Each course brings together people in a similar situation, with support from trained facilitators with lived experience or health professionals. See start dates below.

This Hope Programme is a 6 to 8 session course for people living with multiple long-term conditions that you can start straight away. There is no waiting and you can explore the sessions at your own pace. Technical support is provided. See course options below.
All Hope Programme courses include ongoing access to course content and the Hope online community after the course ends. This means you can stay connected with others, join events, and continue to manage your health.

Self-management means taking steps to look after your health, especially if you have a long-term condition like asthma, diabetes, COPD or depression. It’s about making choices each day that help you feel better, like eating well, staying active, taking your medication, and knowing what triggers your symptoms.
Self-management doesn’t mean doing it all alone. Self-management is most effective when you can work with your healthcare team, join support groups, and get advice from professionals to stay on top of your condition.

Self-management puts you in control. It helps you learn more about your condition, face everyday challenges, and make choices that suit your life. On average, people who have one or more long term conditions spend less than 3 hours a year with their healthcare provider. Self-management is a partnership with your healthcare team and others in similar situation, but it equips YOU with the skills and knowledge to be in the driving seat.

Digital self-management is just like regular self-management, but you use technology to make it easier. On the Hope Programme you can read text, watch videos, do interactive activities, quizzes, track your symptoms, set goals and engage with other people in similar situation through forums and messages. Digital tools can send your reminders and do some of the hard work for you, so you can focus on keeping well.

Anyone with a long-term condition can benefit. Whether it’s a physical condition like diabetes or a mental health condition like anxiety, self-management helps you take control and live life on your terms.

The Hope Programme is for anyone 18 years or older who lives in a postcode in the South West region.

WHAT IS THE HOPE PROGRAMME®
HOPE: Help Overcome Problems Effectively
The Hope Programme is a 6 or 8 week digital self-management course based on positive psychology, mindfulness and cognitive behavioural therapy, built on 25 years of research evidence.
The programme targets people living with any long-term conditions, empowering them to take control of their health and wellbeing.

Hope Programme for the South West is designed to be used on demand.
Participants get to choose what self-management support they need, when they need it.
The support ranges from self-guided personalised e-learning style resources, digital peer-support courses facilitated by trained facilitators with lived experience, and an online community.
10,000 adults with a range of long-term conditions can benefit from the support.

Over 33,000 people with a range of mental and physical health conditions have benefited to date.
Robust evaluation: Several studies have shown that the Hope Programme courses - improve positive mental wellbeing, confidence to manage, gratitude and hope whilst also reducing depression and anxiety. Improving quality of life and patient activation thus reducing the burden on healthcare services.
Evidence-informed: Hope Programme self-management resources are based on positive psychology, mindfulness and cognitive behavioural therapy, built on 25 years of research evidence at Coventry University

PARTICIPANT CRITERIA
18 years or older - living in a postcode within South West region
Able to use and have access to a PC, laptop, tablet or smartphone. Internet access is required.
The programme will be most suitable to those who are unable to commit to set times/days and those who may have barriers to being “seen”. Or those for whom privacy issues may be a barrier to taking part in in-person or virtual programmes. The platform enables participants to protect their identity and complete the course anonymously by selecting an avatar. They can also choose how much information they share and with whom.
