We are an award-winning social enterprise dedicated to empowering people living with long-term conditions and their carers to take control of their health and wellbeing. We are driven by our vision for everyone to have the tools to lead a hopeful and flourishing life. Our goal is to revolutionise healthcare through accessible, innovative, scalable evidence-based self-management programmes. We have empowered nearly 30,000 people through our evidence-based self-management Hope Programmes to date, working alongside the NHS, Macmillan Cancer Support, Fire Fighters Charity, and other partners.

We are proud of what we’ve achieved, especially our rapid pivot to digital delivery during the pandemic, which allowed us to reach even more people. This shift has transformed H4C into a leading digital health business, and our efforts have been recognised as are finalists in this year’s Tech for Good Award by Social Enterprise UK.

Our social enterprise was co-founded in 2015 by a university professor and four community volunteers who lived with or cared for someone with a long-term condition. As we approach our 10-year anniversary, we are excited to embark on a new chapter of growth and impact.

Now, we are looking for Non-Executive Directors (NEDs) and an Independent Board Chair to help shape the future of our social enterprise as we plan for the next decade. We are looking for passionate people with experience in digital health, business development and a new chair, to join us on our mission to improve the lives of even more people across the UK and beyond.

Recruitment closes 7th February 2025, with a view to the successful candidates taking up the roles from March 2025. Please note that these are non-remunerated roles as we are a social enterprise.
